You can also save time by printing out and completing the patient forms in advance of your appointment. Please bring your insurance card, a photo ID, your current list of medications, and arrive 15 minutes before your appointment time.

Thursday: 8:00 am - 5:00 pm
Friday: 8:00 am - 2:00 pm 

*We cater our hours to school holidays and early release days in nearby school districts.

We are conveniently located on the NW corner of the intersection of 291-South & 150 Highway, directly behind Quick Trip and Arby's, near Raintree Lake & Lake Winnebago.


Your First Visit

Your first visit in our office typically involves any necessary x-rays, entering your medical history and medications, an oral-cancer screening, a periodontal screening, charting all your existing dental work, and a thorough check-up, and cleaning. If you are having dental pain, we will address this first, and may need another appointment to complete your full examination.
